Capsulitis/Foot Joint Pain

Capsulitis is irritation of the tissues surrounding a joint. In the forefoot it often affects the joint beneath the second toe, where repeated pressure can also strain the plantar plate that helps stabilise the toe.

What you may notice

  • Pain beneath a toe at the ball of the foot
  • Swelling or discomfort when walking barefoot
  • A toe that starts lifting, drifting or crossing over

How we assess it

We check the painful joint, toe position and stability, along with footwear and how you load the forefoot. Imaging may be recommended if a tear or another cause of pain is suspected.

Treatment and next steps

Care may include reducing aggravating activity, supportive footwear, padding, taping or orthotics to redistribute pressure. Earlier assessment is useful when a toe begins changing position. Persistent instability may require a specialist opinion.

When to seek care

Arrange prompt assessment for a new toe deformity, rapidly increasing swelling or inability to walk comfortably.
