Chiropody Insurance & Direct Billing: Patient Guide

Locke Minor Foot Surgery & Orthotics offers direct billing for eligible insurance plans. It can simplify submitting a claim, but it does not determine whether your plan pays for a particular service. Your insurer and policy wording decide eligibility, reimbursement and limits.

What should I check with my insurer?

  • Does my plan cover services provided by a registered chiropodist?
  • Do I need a physician’s referral or other paperwork for reimbursement?
  • What portion of each visit is covered, and what annual limit remains?
  • Are procedures, routine care and custom orthotics handled differently?
  • Can the claim be submitted directly, or must I submit the receipt?

There is no referral requirement to book at our clinic. That is separate from an insurer’s rules for paying a claim. Ask about both if you are unsure.

What to bring

Bring the insurer’s name, member or certificate number, policy details and any required documents. If you have more than one plan, ask how coordination of benefits works. Tell the team which plan is yours and which belongs to another policyholder.

University and college plans

Student benefits, including McMaster and other university or college plans, vary by school, student group and coverage period. Being enrolled at a school does not by itself confirm that a service is covered. Contact your plan administrator before the appointment, especially if you opted out of benefits or have separate family coverage.

Custom orthotics

Orthotics may have separate prescription, provider and documentation rules. Confirm these before ordering rather than assuming the same rules as a routine visit. Our orthotics information explains the assessment discussion.

If a claim cannot be processed

Ask the team what information or receipt you will need to submit the claim yourself. A declined electronic claim does not explain every policy decision; your insurer can clarify the reason. Confirm fees and your payment responsibility before treatment.
